A study investigated the impact of extrahepatic organ failure (EHOF) characteristics on the prognosis of acute-on-chronic liver failure (ACLF) patients. The study included 736 ACLF patients from six hospitals in China. Results showed that the number and type of EHOF, but not the onset time or sequence, influenced patient mortality. Patients were stratified into risk subgroups based on EHOF characteristics, showing differences in mortality risks. Specifically, patients with multiple EHOF and brain failure had a worse prognosis. This study provides valuable information for risk stratification and clinical practice in managing ACLF patients.
