A report analyzed over 100 cases, revealing failures in patient care, including lack of communication, failure to inform families about discharge, and inadequate assessment of requests to leave hospital. These failures can lead to poorer outcomes, including an increased risk of suicide. The report highlighted specific lapses in the cases.
