Julian was born in Horsham and brought up in Crawley, West Sussex. His father (who was blind) was an organist, piano teacher, and choirmaster, and his mother a magistrate. He attended Worthing High School for Boys and won a scholarship to Edinburgh University, starting medical school in 1950. He completed an intercalated honours degree in neuroanatomy and qualified in 1957.
During his academic career he was much involved with politics and became president of the university Labour Club. He completed his preregistration year at East Fortune Hospital, Lothian, and St Bartholomew’s Hospital in Rochester, Kent.
