Semaglutide, a diabetes and weight-loss drug, has been shown to improve heart failure-related symptoms and physical limitations in patients with obesity-related heart failure and preserved ejection fraction, regardless of diuretic use. The drug has benefits in reducing daily loop diuretic dose, preventing new loop diuretic initiation, and increasing discontinuations. However, there are still some uncertainties regarding its efficacy and safety after 1 year, potential renal or other side effects, and its effects on different ethnic groups. The mechanism by which semaglutide affects plasma volume and cardiac function is also not clear, and further research is needed to fully understand its impact.
