New trial data shows that early blood pressure reduction has a positive effect in hemorrhagic stroke and a negative effect in ischemic stroke. This challenges current guidelines on blood pressure control in acute stroke patients. The INTERACT4 trial in China involved 2404 patients with suspected acute stroke, where immediate blood pressure control was tested during ambulance transport. Results showed a decrease in poor outcomes for hemorrhagic stroke patients but an increase for ischemic stroke patients. These findings suggest the need for early blood pressure control for hemorrhagic stroke and caution in lowering blood pressure for ischemic stroke patients before thrombolysis. The study highlights the importance of accurate diagnosis and tailored treatment based on stroke type.
