Methotrexate (MTX) is a treatment option for atopic dermatitis that is moderately effective and well-tolerated, recommended by the 2022 European EuroGuiDerm guideline. Dosing guidelines for adults range from 5 to 15 mg/wk initially, while for children, it is 0.3 to 0.4 mg/wk. A panel of dermatology experts developed consensus recommendations for MTX use, including weekly administration, gradual dose increases, and switching to subcutaneous dosing for gastrointestinal side effects. No maximum treatment duration is specified, and monitoring cumulative dosage is unnecessary. Adults typically start at 15 mg/wk, with a maximum dose of 25 mg/wk, while children are dosed based on weight. Administration route preferences remain neutral between oral and subcutaneous.
