Debate Settled on Preop Chemoradiation in Gastric Cancer?

The TOPGEAR study investigated the efficacy of adding preoperative chemoradiation to standard perioperative chemotherapy for resectable adenocarcinoma of the stomach or gastroesophageal junction. The study found that the addition of preoperative chemoradiation did not improve overall or progression-free survival. Despite this, preoperative chemoradiation did increase pathologic complete response rates and tumor downstaging. While the study did not meet its primary endpoint, it demonstrated potential benefits for certain cases where reducing tumor load is necessary. The results suggest a limited but significant role for preoperative chemoradiation in the treatment of gastric cancer.
