The program offers personalized support, guidance, and data collection for patients, aiming to streamline care and improve outcomes. The initiative is designed to address the challenges of treating AFib, a common form of cardiac arrhythmia often accompanied by other health issues. Story Health’s focus on human connection and experience in heart failure care made them an ideal partner for Saint Luke’s. The program will provide ongoing support and guidance for AFib patients starting in June.
