First-trimester use of antiseizure medications (ASMs) does not increase the risk for miscarriage, according to a UK population-based cohort study. The elevation of risk among users is likely due to underlying psychiatric conditions for which the medications are used. Overall, 14.5% of pregnancies with first-trimester ASM exposure ended in miscarriage, compared to 12.2% without exposure. Risk was higher for women using ASMs for bipolar disorder or other psychiatric conditions, but not for epilepsy. The study suggests that previous associations between ASMs and miscarriage may be due to other factors. This information can help inform women considering or currently using ASMs during pregnancy.
