The FRESH-UP trial compared fluid restriction with liberal fluid intake among patients with chronic heart failure.

The trial included 504 adults with NYHA class II/III HF at least 6 months before enrollment. Patients were randomly assigned to fluid restriction of no more than 1,500 mL per day or liberal fluid intake with no restriction.

Over 3 months, change in health status as assessed by Kansas City Cardiomyopathy Questionnaire overall summary score (KCCQ-OSS) favored liberal fluid intake vs. fluid restriction intake; however, the difference was not significant.

Patients assigned to follow restricted fluid intake reported higher thirst distress compared with those assigned liberal intake.

KCCQ clinical summary and KCCQ total summary scores were higher in the liberal fluid intake arm.

The FRESH-UP trial was presented at the 2025 American College of Cardiology Scientific Session and simultaneously published in Nature Medicine.
