In the double-blind INSPIRES trial, patients with mild ischemic stroke or high-risk TIA were randomly assigned to receive either clopidogrel plus aspirin or aspirin alone within 72 hours of symptom onset. The study found that the combination of clopidogrel plus aspirin reduced the risk of new stroke by 2 percentage points compared to aspirin alone, with a lower risk of composite cardiovascular events and ischemic stroke as well. However, moderate to severe bleeding risk was higher in patients who received the combination treatment. The study’s corresponding author was Yilong Wang, MD, PhD, and it was published in the New England Journal of Medicine. Limitations include the exclusion of certain patient groups, and the study was supported by grants from various sources.
