Alitretinoin is more effective than phototherapy in quickly reducing severe chronic eczema of the hands, with better compliance but higher rates of adverse events. A study on 441 patients found that at 12 weeks, alitretinoin led to a 30% improvement in eczema severity compared to 50% with phototherapy, and was more cost-effective. Both treatments had similar rates of clear skin achievement, but alitretinoin had better compliance. However, alitretinoin was associated with more adverse events. The study highlights the need for improved access to effective treatments for severe hand eczema. Limitations include potential bias and difficulty comparing long-term efficacy.
