A large rheumatology clinic in Richmond, Virginia, launched a virtual navigator program after learning that Medicare would reimburse patient navigators. This initiative aims to provide assistance to patients with high-risk conditions, including rheumatologic diseases. CMS established reimbursement codes for navigators in 2024, recognizing the importance of patient support in improving health outcomes. Studies have shown the effectiveness of navigators in reducing barriers to care and improving patient satisfaction. Various programs, such as those at the Lupus Foundation of America and SUNY Downstate, have successfully implemented navigator services to support patients with rheumatic diseases. VA also utilizes navigator services to enhance care coordination for veterans with complex rheumatologic conditions.
