integrates clinical care of the motor and non-motor symptoms with a referral process
for physical therapy and for addressing occupational and social issues, particularly
safety issues about the home. To aid this follow up, it would be helpful to have a
concise reference of community-based resources from which selected information can
be provided to patients during their clinical visits. A reference guide of this sort
(Table 1) will be of particular help to clinicians without access to the ancillary support
services found at a university or major medical center.
