Optimal anesthetic strategy for endovascular stroke therapy: Begging for a good trial

Since the first observational studies suggested that the choice of anesthetic strategy could substantially affect the outcomes of patients with acute ischemic stroke undergoing endovascular therapy,1 intense research has focused on the question of which is the optimal anesthetic approach during mechanical embolectomy.2 This research, typically comparing conscious sedation (CS) and general anesthesia (GA), has yielded conflicting results. Consequently, current guidelines acknowledge a state of equipoise between these 2 options.3
